All Applicants

Please bring the following items with you when you are invited for an interview:

Passport
Relevant Work Permits (If applicable)
National Insurance Number (Card/P45/P60)
Birth Certificate
Enhanced Criminal Records Certificate (CRB)
Driving Licence (If applicable)
Immunisation Record (from your GP) to include written proof of hepatitis B within the last five years
Proof of any Current Courses Attended (NVQ, C&R, M&H, First Aid etc.)
Bank Details
Proof of Current Address

Qualified Nurses

If you are applying to work in your capacity as a qualified nurse, you will also need to have the following item(s) in your possession:

Statement of Entry in the Professional Register NMC / UKCC Registration Card (showing expiry date)
